In order to assure that your visit and/or experience is optimal, we recommend that you familiarize yourself with the following guidelines.
Preparing for Your Appointment
In order for us to provide the best care for you, it is very important that you bring the following items to your visit:
Photo ID
A list of your current medications
Insurance card
Information about what treatments and medications have been tried in the past
Medical records from prior visits that you think may be helpful
Insurance provider policies state that you cannot be seen by a physician without presenting a valid Insurance ID card.
If you are a new patient to our clinic prior to your appointment complete the New Patient Information Packet and bring to your appointment.
At Your Appointment
We ask that all New Patients arrive 30 minutes prior to their appointment time and established patients should arrive 15 minutes prior, in order to allow our staff to make copies of your cards and collect co-payments.
Financial Responsibility
On the day of your appointment, we will collect your co-payment, co-insurance or deductible (or any portion not already met) due for services received that day. New OB patients will meet with a staff member to discuss your payment options.
Your doctor may provide a service based on his/her professional judgment at the time of your visit that your insurance company will deem a “non-covered benefit”. If your insurance company sends you a letter stating “non-covered benefit” please call them for further explanation.
